Consent & Release Form 

This form is for your groomer to have details of your pet on file. If you have more than one pet please fill out one form for each of your pets so they have their own notes. The consent & release form will be renewed (once) every January for repeating clients to update any changes in your pet(s) as well as the contact information. This information will not be shared with anyone other than Grooming Woof Love.
